There are two players, Wife / girlfriend (= W) and Husband / boyfriend (= H). The goal of the game for W is to get away with an affair or find a better partner, for H is to succeed in his career and prevent infidelity.

There are three outcomes: 1, Netorare (W win), 2, Netorase (Dual win) and 3, Vanilla / marriage (H win)
If the outcome is 1 or 2, then W doesn't have to reveal her game state.

Gameplay setup & accessories:
The board has a large number of locations. W has to declare her starting location by placing a large figurine on the board and her secret location by a turned down location card. Alternatively, by placing her small token behind a location door while H closes his eyes. W could also mess with the board state depending on certain criteria.
Both players have their own deck of cards, a separate set of world event cards and a set of job assignment cards. (I.) Player cards further their own unique goals and mess with the other's. For example, W could shuffle or remove cards from H's deck, or H could temporarily restrict W's actions for a day. Player cards can include encounters with attractive co-workers, going on dates, erotic gifts, locking map districts, overtimes, relaxing baths, etc. (II.) World cards effect the world state for the day or week. Events can include item sales, location renovations, pandemics, vacations, specific holidays, overtimes, business trips, rain, etc. (III.) Job assignment cards contain tasks the players have to finish in a certain number of rounds. H's jobs are harder and more time sensitive, W's are more lenient and require stopping at more locations, while having a chance for adventurous encounters at each.

The large map is divided into districts and locations, interconnected by shortcuts like metro stations. In each phase the players can move at most 2 distance away from their current location. At the start of the day both players put their player tokens at their starting location, with W also designating a secret location.

During the day H can either focus on his assignments or check-up on W (= stalk). H can't afford to stalk often else he fails his assignment and won't gain career points. Time inherently favors W, so H can't just stall the game.
W can also either focus on her assignment or actively seek encounters, pursuing one of the many male NTR interests (= Bulls). W does this by rolling for her secret location, instead of the current one tied to her assignment. W has a regular chance to be hit on or flirted with during her assignments modified by location, giving her baseline contact with Bulls. If they meet, they must greet each other and W has to explain what is she doing there. If W was up to no good and was rolling for affairs, she loses progress for the whole day and with the guy she was meeting.

Give both the mc and the heroine agency and make their love sincere in a monogamous relationship, show her and the mc being vulnerable while fighting for each other, show that they are not afraid to love. Most gacha and harem stories get hit heavily with NTR doujins exactly because of how insincere the love between the mc and the heroines feel in those stories. And this happens exactly because authors want to milk those stories for as long as possible.

Discussing this topic online is pointless because every NTR hater is convinced that their personal reason for why people enjoy NTR is the only correct one and anything else is cope, denial, or outright lying.
