Obviously not to raise suspicion upon letting them in and to make you go insane and terrified which is why they only start to kill actual ponies once two visitors are in the house. If all ponies are killed except the player then Celestia will come in and kill you basically.

Inspection, as seen in the video with Pinkie Pie's eyes getting checked and it was red which clearly meant something wrong aka she's a visitor. Darling Do likely had her hooves wrong but her eyes are not fine so its a bit of a guessing game and a 50/50 in the inspection part.
