apple removes all the pictures from your phone when you're not using it so they can check to see if you're not violating any laws or their terms of service (and to copy all the good ones). sometimes they forget which albums they were in and put them back wrong.

Might be a sync issue, my dad had an older iphone and when he upgraded it took like 3 days for things to move properly.
I'd look into things deeper, but I've got a Samsung so that's not happening.
