This wasn't a wide-net update hack, this was targeted. Any AV program wouldn't have picked it up.

Meanwhile common sense 2.0 told me that I haven't needed to update Notepad ++ in four years nor give it access to the internet, so I haven't.

heuristic analysis might work but it's still flawed. We're going to see a lot more of it coming out in the next couple years though.
