There is nothing really differentiating male attracted mtfs from regular gay males in terms of brains.
That said though, there is evidence gay males in general were feminized in the womb compared to regular males.
That's why you have the fraternal birth order effect for example, and why castrating mice when they're young produces female behavior.

For people with gender dysphoria, no matter the sex, their insular cortex can’t communicate properly; specifically the part of the brain that signals “body-identification”. Basically, for people with GD, it seems like the brain can’t quickly recognize the body in its current state as “the self” in the way that cis people can - more conscious thought has to go into it.
There are some other disorders where this happens too, so this unfortunately could never work 100% for diagnosis. However, HRT has, so far, only been shown to fix those communication issues for people with gender dysphoria. So it’s most likely one of those disorders that are only truly diagnosable by if the meds improve things or not.
There have also been studies that show that they do more quickly recognize the body as “the self” once it has (or is shown to have) sex characteristics associated with their gender.
There’s a lot we still don’t know and is still being studied - but it is an observable experience!

Also fraternal birth order effect was observed in humans. The more older male brothers you have the more likely you are to be gay since women develop more of an immunity against male fetuses each time they have a boy. It helps explain one reason why gay (femininzed) males exist.
