Imagine a world where fatties wind up wearing undersized clothing not because it's hot to be too fat for your entire wardrobe, but because food/housing/transit/healthcare/food/food/food is incredibly cheap and affordable and wages are more than livable, yet clothing with double digits in front of the "extra large" is really expensive.
It becomes much more acceptable for someone to show up to work like this when it's because getting a shirt a few sizes larger would cost more than a week's meals.

Of course, it becomes so common as to be normalized and accepted, just as much as indulging in 10,000+ calories of fast food a day. It almost becomes a status symbol for a fatty to always have clothing on hand that actually fits in a traditional sense. Tight, form-enhancing, certainly not immune to zipper and button integrity issues, but that entire gut is covered. For now.
