Honestly San Andreas' weight gain mechanic works well because it's so totalizing. When CJ gets fat his stamina goes down, he gets tired more easily, his walk/waddle is slower, and his dialogue in particular circumstances (like when you're fucking a hooker) becomes very food- and fat-centric. This always feels like an under-utilized theme for WG. Most Harleys, especially without the Joker, don't make a habit of going after randos so that feels fitting. I love the idea of Oracle going full addict being stuck by herself and eventually giving into her gluttony and accepting Harley as her new ideal, Her snack cake addiction and a healthy dose of denial that she could have stopped this distorting her reality so she just gives into being a co-conspiritor.
At first she tries to play both sides of the field, keeping her out of control weight and obsession with Harley's food to herself while helping the others with whatever they ask of her in their efforts against Harley. But the more time passes, her weight keeps ballooning, and her addiction gets stronger. She starts doing work for Harley, helping her with cyber skills the one thing Harley lacked. And eventually gets so deep into Harley's bad influence that she's undermining the girls. Without Harley even directing her to she's setting traps for her friends and keeping them just close enough to think they're on the right track without ever letting them get too far, all the while trying to push them into addiction too.

I think the Titan is just for her and her henchgirls(and Harley got the effects permanently due to her weird bodychemistry from a Joker dunk and then Ivy).

It's basically after Joker's death and months of stresseating and psychosis, Harley adopts the persona of a obese circus fat lady and ringmaster called Big Top Harl, who quickly uses her genius mind and raw might to do the Red Hood strategy of taking over all the crime in the city by strongarm and actually lowering crime by driving the others out of her turf.

She succeeds in catching Batman off guard and crushing him to death, and thus quickly expands her influence and herself. People in her territory find gyms and salad bars burned down while food is dirt cheap and Harley runs a bunch of local charity programs to end hunger and ensure free fitting clothes for everyone. (They broadly know they're gonna get fat, but it's way better than anything else any other crime boss in Gotham would offer and so they don't care). Harley has that Cheesecake mindset she's not super subtle.

Plot would follow either the Batgirls trying to stop Harley while threading on the edge of temptation themselves, or female supervillains having to deal with Harley, either working with her willingly or trying to work against her and having Harley use her full force of scheming and tricks to try to get them to see things her way and get them as fat as possible.
