You have three separate perks, each one representing one of diesease stages.
>HIV
Nothing, you're noted by console prompt (black-green thingy in left bottom corner) that you contracted HIV, and in character sheet there's a karmic perk that says the diesease will probably get worse over time.
>AIDS I
Another prompt that you're constantly feeling unwell. Your actual stats drop (penalty to Endurance). And karmic perk is replaced with second one.

Another prompt that say's you are permanently sick and must begin searching for treatment. Karmic perk is replaced with third one that describes that you have some skin sickness, it gives penalty to Endurance and Charisma.

I dont remembre exact numbers of these penalties, but I think it was like -2END, then -4END, -2CHA. My party members didnt left me and I had 8 CHA in chargen, so it could possibly be either -1 or -2.
Even final stage surprisngly doesnt kill you, but it's curable. Neat little hidden quest.
